FAQs
Can I request Montgomery County Home Health by name?
Yes. Patients in a hospital or skilled nursing facility must request Montgomery County Home Health to avoid being discharged to another home health care provider. We would love to be your Home Health Care Provider!
How often will my home health visits be?
The number of visits you receive will be determined by your doctor’s orders in your personal plan of care. Your plan of care may be changed to increase or decrease the number of visits, depending on your personal need.
What’s the difference between home care and home health care?
A Home Health Care Provider, such as Montgomery County Home Health, is necessary when there is a medical need that requires a health care professional to assist in helping you regain your quality of life. It may also be prescribed by a doctor when you are suffering from a chronic condition such as diabetes or COPD. A doctor will determine if home health care is necessary. Your plan of care can include some personal care services, such as bathing and dressing.
A Home Care Agency provides household and personal care duties, such as cooking meals, cleaning, and assistance with personal care (i.e. bathing or dressing). Montgomery County Home Health does NOT provide these services.
